All organizations certified under R. 1:21-11 must complete an annual recertification each year by April 30th, starting in 2016, as per R. 1:21-11(b)(ii). Please select the selection below that meets your organization’s or program’s current status.

By selecting this, I am providing notice, which will be sent to the Clerk of the Supreme Court, that the above-referenced organization or program no longer meets the definition in R. 1:21-11(a)(2) or (3).

Please note that an organization or program that fails to timely file its yearly certification under R. 1:21-11(b)(1)(ii) will lose its status as a certified entity under subsection (b)(1).

List of Attorneys: Whether or not your organization continues to meet the definition in R. 1:21-11 (a)(2) or (3), if your organization used attorneys who provided qualifying pro bono service during the previous year, you will need to submit a list of those attorneys.

After you have clicked the above certification button, you then will need to submit the names and attorney identification numbers of these attorneys (if the ID numbers are readily available). Attorney identification numbers can be easily obtained from the Attorney Index; click here to access the Attorney Index. The list of attorneys should be submitted in Excel format. The Excel template can be downloaded by clicking here. Once you have downloaded the template, you may then type in the attorney names and identification numbers and save the completed template to your computer.

To upload that prepared list of attorneys (the completed template) to the recertification application form, you first locate that document on your computer using “browse.” When you have found the document (the completed template setting out the list of attorneys), click “open”, at which point the file will automatically load to the “file upload” box. Please note: the window for “file upload” will appear after you click the button above indicating you are certifying the list of attorneys. Hitting the “submit” button at the end of this form will upload the list of attorneys to the recertification application and will submit the application.
