Effective September 5, 2000, Rules 1:5-6 and 4:5-1 require that all initial pleadings filed in the Superior Court, Law
Division, Civil Part must be accompanied by a Case Information Statement (CIS) in the form prescribed in Appendix
XII to the Rules of Court. Please note that the CIS form in the Appendix has been amended, also effective September 5,
2000. All appropriate pleadings submitted for filing, therefore, should be accompanied by the CIS as recently amended.
Pleadings accompanied by the CIS form in effect prior to September 5, 2000 may be returned stamped "Received but
not Filed."
