Revised Appellate Division Notice of Appeal and Criminal Case Information Statement Forms
The Supreme Court and the Acting Administrative Director of the Courts have approved the revised Appellate Division
notice of appeal and criminal case information statement forms. The revised forms, reflecting a May 2000 date, are
included as part of this notice. They replace the prior versions set forth in Appendix IV and VIII of the Court Rules.
Pursuant to R. 2:5-1(f)(1), the use of the notice of appeal form is optional. Attorneys may start using the revised form
immediately.
Pursuant to R. 2:5-1(f)(2), the use of the criminal case information statement form is required. Attorneys should start
using the revised form immediately.
