The Supreme Court and the Acting Administrative Director of the Courts have approved the revised Appellate Division notice of appeal and criminal case information statement forms. The revised forms, reflecting a May 2000 date, are included as part of this notice. They replace the prior versions set forth in Appendix IV and VIII of the Court Rules.
Pursuant to R. 2:5-1(f)(1), the use of the notice of appeal form is optional. Attorneys may start using the revised form immediately.
Pursuant to R. 2:5-1(f)(2), the use of the criminal case information statement form is required. Attorneys should start using the revised form immediately.
