Supplement to Directive 15 18 Family Children in Court CIC Revised CIC Permanency Form Orders CN 10255 CN 10259 CN 11399

This Supplement to Administrative Directive # 15-18 promulgates revisions to three Children in Court (CIC) permanency order forms (CN 10255, CN 10259, and CN 11399) effective immediately, as approved by the Judicial Council and as posted in the forms catalog on the Judiciary website. The approved revisions to the three form orders are as follows:

• The addition of checkboxes to indicate that a review under the Child Placement Review (CPR) Act was performed by a CPR Board or the court (FC Permanency Order (CN 10255);
• The addition of a dropdown menu listing reasons why a child did not appear at a permanency hearing (FC Pennanency Order (CN 10255), Permanency Order - FN & FG Cases (CN l 0259), and FC Post Termination Permanency Order (CN 11399)
• Revised confidentiality notice clarifying that the prohibition of sharing confidential information includes electronic communications and social media (FC Permanency Order (CN 10255), Permanency Order - FN & FG Cases (CN l0259), and FC Post-Termination Permanency Order (CN 11399).
